Die Dermatitis digitalis (DD) des Rindes wurde erstmals Mitte der 1970er-Jahre in Italien detektiert und beschrieben. Seitdem breitet sich die Erkrankung sowohl in Europa als auch international aus. In den vergangenen zehn Jahren wurde nach langer beschriebener Freiheit die Erkrankung auch in Chile und Neuseeland nachgewiesen. DD hat sich somit zu einem internationalen Problem rinderhaltender Betriebe entwickelt. Die momentanen Forschungsergebnisse weisen die zu den Spirochäten gehörenden Treponemen, als primären Erreger der DD aus. Ziel dieser Studie war es, den Einfluss antibiotischer und nicht antibiotischer Behandlungen auf M2-Läsionen der DD klinisch zu erfassen und über Stanzbiopsien die Wirkung auf die Erreger in den Gewebeschichten sowie die Regeneration des betroffenen Gewebes histologisch zu vergleichen, um eine mögliche Elemination oder Reduktion der Erreger feststellen zu können. Für die Datenerhebung wurden insgesamt bei 105 Tieren von fünf bayrischen, Milchviehbetrieben, jeweils eine M2-Läsion (>2cm) mit einem von fünf antibiotischen und nicht-antibiotischen Produkten behandelt. Abschließend befanden sich jeweils 21 Tiere in jeder Behandlungsgruppe. Jede Behandlungsgruppe war sowohl aus Typ2 und Typ3-Kühen zusammengestellt. Die Versuchsphase auf den Betrieben erstreckte sich von August 2016 bis Juni 2017. Neben der klinischen Verlaufskontrolle wurden an Tag 0, 12 und 28 Stanzbiopsien (Ø 6mm) unter lokaler Anästhesie des Wundbereichs entnommen. Die Proben wurden halbiert und sowohl der Histologie, als auch der Dunkelfeldmikroskopie für ein Auswanderungsverfahren der Treponemen zugeführt. Die Auswertungen der M-Stadien zeigen deutliche Unterschiede in dem Heilungsverlauf der Studiengruppen hinsichtlich der M2-Reduktion, Entwicklung von M4.1-Läsionen und Ausbildung chronischer Krankheitsstadien. Die Produkte wurden in der statistischen Auswertung jeweils in einen Vergleich zu einer Chlortetracyclin-Spray-Gruppe gestellt. In der histologischen Auswertung sind statistisch signifikante Unterschiede zwischen den Behandlungsgruppen bei der Reduktion der Treponemen aus den tiefen Gewebeschichten, dem intakten, langfristigen Wundverschluss, sowie den frühen Anzeichen einer chronischen Entzündung herausgestellt worden. Die frühen, histologischen Anzeichen einer chronischen Entzündung korrelieren hierbei mit der makroskopischen Bewertung einer proliferativ veränderten Läsion. Zusammenfassend war festzustellen, dass wirksame Produkte ohne Antibiotika für die oberflächliche Behandlung akuter Läsionen der DD auf dem Markt vorhanden sind. Zwar kann keine Heilung mit den in die Studie einbezogenen Medikamenten erreicht werden, aber auch die eingesetzte Antibiose vermochte dies nicht zu erfüllen. Es ist möglich, eine akute, schmerzhafte Läsion mit nichtantibiotischen Produkten zu behandeln, um in kürzester Zeit eine schmerzfreie, verschlossene Läsion zu erreichen. Diese Erkenntnisse werden durch die Auswertung der Histologie unterstützt. Zwar kommt es nicht zu einer vollkommenen Erregerfreiheit, aber es wird die Möglichkeit der Regeneration der Gewebeschichten nach dem Verschluss der Wunde erreicht., New ways to preserve claw health - comparative study on the efficacy of antibiotics and antibiotic-free treatments for digital dermatitis (Mortellaro disease) Digital dermatitis (DD) is an international problem on beef and dairy farms and was first described in 1974. Damaged and infected skin shows erosive and painful open lesions. The typical lesion is located on the hind legs at the coronary band. More than 80 % of German dairy farmers have to deal with DD on their farms. Due to the high number of infected animals, a lot of drugs are in use. Tetracycline spray is a commonly used topical treatment containing antibiotics, that is used in the milking parlor or while hooftrimming. After topical treatment is judged effective by positive clinical evaluation many relapses occur throughout the herds, supposedly caused by encysted Treponema in the epidermal layers. To reduce the amount of dispensed drugs it is necessary to find an effective, pain reducing and sustainable treatment without a large pharmacological input into the environment. The superficial treatments are compared in this study. An antibiotic (chlortetracycline spray), different antibiotic-free treatments (salicylic acid, polyurethane bandage, copper-chelate spray) and a combination (chlortetracycline spray + polyurethane bandage) are used. In preparation for the study all data on claw health was collected on five Bavarian dairy farms during the routine trim to get the individual disease history of every cow. Based on this dataset the cows were randomly split into five different treatment groups and an even number of type-2 and type-3 cows added up to 21 cows per group. The on farm work of the trial was accomplished from August 2016 until June 2017. The study is innovative in performing three biopsies on the lesions during the healing process on day 0, 12 and 28 under local anesthesia and comparing the success of each treatment clinically and histologically. This way, not only the clinical effect on the surface of the lesions, but also the effect on the bacteria in deeper tissue layers can be evaluated. The main questions to be solved in this study are whether antibiotic-free products have the same positive effect on the eradication or reduction of bacteria as an antibiotic treatment and is there an effect of a superficial treatment of DD in the deeper layers of the skin? Each biopsy was split in half for performing histology and dark field microscopy. The evaluations of the M-stages show big differences in the ability of eliminating M2-stages, the development of M4.1-lesions and the outcome of chronical signs. For the statistical analysis each treatment group was compared to the antibiotic treatment (ctc-spray). There were statistical significant differences between the groups in the reduction of Treponemes in the deeper skin layers, a proper wound closure over time and the development of early signs for a chronic inflammation. These early signs of chronic inflammation were correlated to the proliferative lesions from the macroscopic scoring with the M-stages. The results of the study show, that there are effective non-antibiotic treatments available for DD without the risk of antimicrobial resistance. Neither one of the antibiotic free products nor the antibiotic product have shown the efficacy to actually cure DD. The antibiotic free products show the ability to eliminate pain and to close the wound quickly. This statement is supported by the histological findings. There is no total eradication of the Treponemes from the tissue, but the possibility of tissue regeneration beyond the wound closure.
