Zur Morphologie der physiologischen Foveae am Augenhintergrund von Greifvögeln und Eulen mittels Optischer Kohärenztomografie (OCT)
Die Optische Kohärenztomografie (OCT) ist eine neue Methode der Bilddarstellung, welche bereits erfolgreich in der humanmedizinischen Ophthalmologie etabliert wurde. Sie bietet die Möglichkeit, den Aufbau und die Morphologie der Retina in Querschnitten darzustellen und dies nicht-invasiv, kontaktfrei und in vivo in annähernd histologischer Auflösung. In der aviären Ophthalmologie wurde die OCT bisher kaum angewendet. Das Ziel der vorliegenden Arbeit war es daher, die OCT als ein geeignetes Diagnostikum zur Untersuchung der Fovea zu bewerten. Hierfür lag der Fokus dieser Arbeit auf der Darstellbarkeit und Darstellung der physiologischen Fovea bei Greifvögeln und Eulen. Morphologische Variationen der beiden Fovea-Typen F. centralis und F. temporalis innerhalb der Spezies wurden untersucht, ebenso wie Variationen innerhalb der und zwischen den nacht- und tagaktiven Vogelgruppen. Dazu wurden in einem Zeitraum von 1,5 Jahren 56 tag- und nachtaktive Wildgreifvogelpatienten 13 verschiedener Arten aus drei Ordnungen untersucht. Die Tiere stammten alle aus dem Patientengut der Klinik für Vögel, Reptilien, Amphibien und Zierfische der Ludwig-Maximilians-Universität München. Bei allen Tieren war eine ophthalmologische Untersuchung, an die sich eine Untersuchung mittels OCT anschloss, medizinisch indiziert. Diese wurde in Abhängigkeit von Patientenstatus und Toleranz des Tieres entweder in wachem Zustand, in Sedation unter der Anwendung von Midazolam (2 mg/kg oronasal), oder unter Inhalationsanästhesie durchgeführt. Für die OCT-Untersuchung wurde das Modell Spectralis® HRA+OCT Plus der Firma Heidelberg Engineering und für die Bildbearbeitung und Archivierung das Software-Programm Heidelberg Eye Explorer (HEYEX) in der Version 5.4, Heidelberg Engineering, Heidelberg, Deutschland, verwendet. Von den Foveae centrales und temporales wurden sowohl horizontale als auch vertikale Schnittbilder (B-Scans und 3D-Aufnahmen) angefertigt. Dies beinhaltete Scans der jeweils tiefsten Stelle der Fovea und der parafovealen Eminenzen an den Lokalisationen nasal, temporal, superior und inferior. Zusätzlich wurden zum Vergleich Längs- und Querschnitte eines retinalen Zwischenbereichs angefertigt, welcher im Schnittpunkt des Augenäquators (Verlauf der Gefäßkaskade) und einer gedachten Verlängerung entlang des Pecten oculi lag. Die Messungen beinhalteten im Bereich der zentralen Fovea, also an der tiefsten Stelle, die foveale Retinagesamtdicke, die Dicke der Schichtkomplexe von der inneren Körnerschicht bis zur äußeren Grenzmembran und von der äußeren Grenzmembran bis zum äußeren retinalen Pigmentepithel resp. bis zur Basalmembran. Weiterhin wurde die Tiefe der Einsenkung berechnet und die Retinaschichtung der zentralen Fovea vermerkt. Bei den Eminenzen und im Zwischenbereich wurde die retinale Gesamtdicke (von der inneren Grenzmembran bis zur Basalmembran) und zusätzlich folgende Schichten als Komplexe vermessen: die Nervenfaserschicht und die Ganglienzellschicht, die innere plexiforme Schicht und die innere Körnerschicht, die äußere plexiforme Schicht und die äußere Körnerschicht, die inneren Körnerschicht bis zur äußeren Grenzmembran und die äußere Grenzmembran bis zum äußeren retinalen Pigmentepithel bzw. bis zur Basalmembran. Außerdem wurde bei den Eminenzen der Abstand nasal-temporal und superior-inferior, also die Weite der Foveae an der Retinaoberfläche, vermessen. Mittels OCT konnte die Fovea in beinahe histologischer Qualität dargestellt werden. Es waren Gemeinsamkeiten und auch deutliche morphologische Unterschiede zwischen den verschiedenen Spezies erkennbar. Bei allen Vogelarten konnte eine etwas längliche Form der Fovea gezeigt werden. Die unterschiedlichen Foveatypen, konvexiclival bei den tagaktiven Greifvögeln und konkaviclival bei den nachtaktiven Arten, waren deutlich erkennbar, genauso wie Größenunterschiede sowohl innerhalb der tagaktiven und der nachtaktiven Spezies, als auch zwischen den beiden Gruppen. Bei den tagaktiven Arten war die F. centralis im Vergleich zur F. temporalis der Eulen weiter und tiefer ausgeprägt. Die F. temporalis der tagaktiven Greifvögel war insgesamt weniger deutlich ausgebildet und ähnelte eher der F. temporalis der nachtaktiven Arten. Die Reduzierung der Schichten im zentralen Bereich der Fovea reichte bei der F. centralis der tagaktiven Greifvögel und der F. temporalis der Eulen bis zur inneren Körnerschicht. Bei der F. temporalis der tagaktiven Arten reichte sie nur bis zur Ganglienzellschicht. Die Eminenzen wiesen bei allen Foveae-Typen dieselben Schichten auf. Die Dicke der Gesamtretina und der Schichtkomplexe variierte sowohl zwischen den Arten in der jeweiligen tag- oder nachtaktiven Gruppe als auch zwischen Tag- und Nachtaktiven. So waren die Retinagesamtdicke und die Komplexe bis zur äußeren Grenzmembran bei den tagaktiven Greifvögeln insgesamt dicker als bei den nachtaktiven Eulen. Die Schichten von der äußeren Grenzmembran bis zum äußeren retinalen Pigmentepithel / Basalmembran waren hingegen bei den nachtaktiven Arten dicker. Bei beiden Gruppen waren die innere plexiforme Schicht und die innere Körnerschicht dicker als die äußere plexiforme Schicht und äußere Körnerschicht. Bei den Eulen waren jedoch die äußere Körner- und die äußere plexiforme Schicht relativ deutlich ausgeprägt. Beim Wespenbussard konnten im Unterschied zu den anderen Vogelarten die retinalen Schichten nicht eindeutig zugeordnet werden. Mittels OCT konnte die Fovea von Greifvögeln und Eulen einschließlich des Schichtaufbaus der Retina dargestellt werden. Diese Studie lieferte somit eine erste Grundlage zur Interpretation von OCT-Ergebnissen hinsichtlich der physiologischen Morphologie der Foveae beim Greifvogel und die Voraussetzungen zur Erfassung und Bewertung von Veränderungen in der Fovea., Optical coherence tomography is a new method of imaging that has been successfully established in human ophthalmology. It offers the possibility to display structure and morphology of the retina in cross sections at nearly histological quality in vivo, while also being non-invasive and without contacting the eye. In avian ophthalmology, OCT has so far rarely been used. The aim of this study was, therefore, to evaluate OCT as a suitable diagnostic agent for investigating the fovea. To this end, this study focused on the presentability and presentation of the physiological fovea in birds of prey and owls. Morphological variations of both fovea types—fovea centralis and fovea temporalis—were investigated within the diurnal species, as well as variations within and between the diurnal and nocturnal bird groups. To investigate this, 56 diurnal and nocturnal wild birds of prey, representing 12 different species from three zoological families, were investigated over a span of 1.5 years. The animals were obtained from the Clinic for Birds, Reptiles, Amphibians and Ornamental Fish of the University of Munich. For all animals, an ophthalmological examination was medically indicated and was followed by optical coherence tomography. This was performed, depending on patient status and animal toleration, either while conscious, in sedation using Midazolam (2 mg/kg oral-nasal or IM) or under inhalation anesthesia. For examination, the Spectralis® HRA+OCT Plus from Heidelberg Engineering was used. Image editing and archiving was done using the software program Heidelberg Eye Explorer (HEYEX) version 5.4 (Heidelberg Engineering, Heidelberg, Germany). From the foveae centralis and temporalis, horizontal and vertical cross-sectional images (B-Scans and 3D images) were conducted. This included scans of the deepest spots of the fovea and of the parafoveal eminences at the locations nasal, temporal, superior, and inferior. Additionally, longitudinal- and cross-sections of a retinal comparison area were prepared, coming from the intersection of the eyeball equator (along the blood vessel cascade) and an imaginary extension along the pecten oculi. The measurements in the region of the central fovea, i.e. at the deepest point, consisted of the entire foveal retina thickness, the thickness of the layer complex spanning from the inner nuclear layer to the outer limiting membrane, and the complex from the outer limiting membrane to the outer retinal pigment epithelium resp. to the basal membrane. Furthermore, the depth of the depression was calculated and the retina layering of the central fovea was recorded. In the eminences and in the intermediate region, the retinal total thickness (from the internal limiting membrane to the basal membrane) and the following layers were measured as complexes: the nerve fiber layer together with the ganglion cell layer, the inner plexiform layer and the inner nuclear layer, the outer plexiform layer and the outer nuclear layer, the inner nuclear layer to the outer limiting membrane, and the outer limiting membrane to the outer retinal pigment epithelium resp. to the basal membrane. In addition, nasal and temporal distance, as well as the superior to inferior eminence distance, was measured indicating the width of the Fovea at the retinal surface. Using OCT, we were able to image the fovea in near histologic grade. Similarities and significant morphological differences between species were observable. In all bird species, a somewhat elongated fovea shape was observed. The different fovea types, convexiclivate for the diurnal birds of prey and concaviclivate for the nocturnal types, were clearly distinguishable, as well as differences in size within the diurnal and nocturnal species and between the two groups. In the diurnal types, the fovea centralis was wider and deeper as compared to the fovea temporalis of owls. The fovea temporalis of the diurnal birds of prey was overall less clearly formed and was more similar to the fovea temporalis of the nocturnal types. The layers in the central area of the fovea were reduced up to the inner nuclear layer of the fovea centralis of diurnal birds of prey and the fovea temporalis of owls. In the fovea temporalis of the diurnal types, the reduction was somewhat less pronounced, namely only up to the ganglion cell layer. In all fovea types, the eminences showed the same layers. The total thickness of the retina as well as of the layer complexes varied between the species within the diurnal and nocturnal groups, as well as between diurnal and nocturnal groups. Thus the total retina and the layer complexes up to the outer limiting membrane were thicker in diurnal birds of prey as compared to the nocturnal owls. The complex spanning from the outer limiting membrane to the outer retinal pigment epithelium/basal membrane was, however, thicker in the nocturnal species. In both groups, the inner plexiform layer and the inner nuclear layer were thicker than the outer plexiform layer and outer nuclear layer. However, in the nocturnal species, the outer nuclear and outer plexiform layers were especially pronounced. In the European honey buzzard foveae, the retinal layers could not clearly be distinguished. Using optical coherence tomography, the fovea of birds of prey and owls could successfully be displayed, including the layering of the retina. This study provides an initial basis for the interpretation of OCT results with regards to the physiological morphology of the foveae in birds of prey and basis points for the assessment of changes in the fovea., La tomografía de coherencia óptica (OCT) es un nuevo método de Imagen, el cual ya es entablado exitosamente en la oftalmología humana. La OCT ofrece la oportunidad de mostrar las capas y la morfología de la retina en sección transversal de un modo no invasivo, sin contracción y « in vivo » con una calidad poco inferior a la histología. En la oftalmología aviaria la OCT no se ha usado con frecuencia. La meta de este trabajo a sido calificar la OCT como instrumento diagnostico adecuado para la investigación de la fóvea. El foco principal del trabajo ha sido la posibilidad de representación y presentación de la fóvea fisiológica de las aves de presa y búhos. Se ha examinado la variación morfológica de los dos tipos de fóvea, la F. centralis y F. temporalis en la misma especie y también la variación dentro y entre los grupos aviarios diurnos y nocturnos. Para esto se ha estudiado en un tiempo de 1.5 anos a 56 aves de presa salvajes diurnos y nocturnos de 13 diferentes tipos y 3 especies. Todos los animales pertenecen a la clínica de la universidad Ludwig-Maximilians-Universität en Múnich. En todos los animales que recibieron un examen oftalmológico seguido de un examen de OCT habían indicaciones para tal. Dependiendo del estado físico del paciente y su tolerancia, los exámenes se hicieron despiertos, en sedación con Midazolam (2 mg/kg oronasal) or anestesia de inhalación. Para el examen de OCT se utilizo el modelo Spectralis® HRA+OCT Plus de la Firma Heidelberg Engeneering y para la edición de imágenes y archivamiento el programa software Heidelberg Eye Explorer (HEYEX)en la versión 5.4, Heidelberg Engeneering, Heidelberg, Alemania. De la fovea centralis y temporalis se hicieron cortes de imagen verticales como también horizontales (B-Scans y tomas 3D). Estas contienen scans de la parte mas profunda de la fovea y de la eminencia parafóvea las localizaciones nasal, temporal, superior e inferior. Ademas se hicieron cortes verticales y horizontales de la parte intermedia de la retina cual corta con el ecuador del ojo (en dirección de los vasos sanguíneos) y una línea imaginaria a lo largo del pekten, como uso de comparación. Las medidas implican en la parte central de la fóvea, quiere decir en la parte mas profunda, el grueso retinar completo de la fovea, el grueso del complejo de las capas de la capa granular interior hasta la membrana limite exterior y desde la membrana limite exterior hasta el epitelio pigmentario retinar exterior respectivamente hasta la membrana basal. También se calculo la profundidad del hundimiento y se anotaron las capas de la fóvea central. En la eminencia y en la parte intermedia se midió el grueso retinar completo (desde el limite de membrana interior hasta la membrana basal) y aparte las siguientes capas como complejos: las capa de fibra de nervio óptico y capas de las células ganglionares, la capa plexiforme interior y capa granular exterior, la capa granular interior hasta la membrana limitante exterior y la membrana limitante exterior hasta epitelio pigmentario de la retina exterior respectivamente hasta la membrana basal. Además se midió en las “Eminenzen” la distancia nasal-temporal y superior-inferior, como también lo largo de las fóvea en la superficie de la retina. Atravez de la OCT se pudo representar la fóvea en calidad cercana a la histología. Se pudo mostrar parecidos y también claramente diferencias morfológicas entre las diferentes especies. En todos los tipos de aves se pudo mostrar una forma alargada de la fóvea. Los diferentes tipos de fóvea, convexiclival en las aves de presa diurnas y concaviclival en las especies nocturnas, se pudieron mostrar claramente, como también la diferencia de tamaño en dentro de las especies diurnas y nocturnas y también entre los dos grupos. En las especies diurnas la F. central eran mas anchas y profundas en su dimensión comparadas a la F. temporal de los búhos. La F. temporal de las aves de presa diurnas no estaban claramente desarrolladas y se parecían mas a las F. temporal de las especies nocturnas. La reducción de las capas en la parte central de la fóvea alcanzaba en la F. central en las aves de presa diurnas y en la F. temporal en los búhos hasta la capa granular interna. En la F. temporalis de las especies diurnas solo alcanzaban hasta la capa de las células ganglionares. Las “Eminenzen” mostraban en todos los tipos de fóvea las mismas capas. El grueso de la retina en su totalidad y las capas complejas variaban entre las especies de los grupos diurnos y nocturnos, como también entre diurnos y nocturnos en la misma especie. El grueso total de la retina y de los complejos hasta el limite de la membrana exterior era mas grueso en las aves de presa diurnas que en los búhos nocturnos. Las capas desde el limite de la membrana exterior hasta el epitelio pigmentario exterior / membrana basal de la retina eran por lo contrario mas gruesos en las especies nocturnas. En ambos grupos la capa plexiforme interna y la capa granular interna eran mas gruesas que la capa plexiforme exterior y la capa granular exterior. En los búhos la capa granular y plexiforme exterior estaban claramente mas desarrolladas. En el halcón abejero no se pudo clasificar claramente las capas de la retina como en las otras especies de aves. Por medio de la OCT se pudo mostrar la fóvea de las aves de presa y los búhos incluyendo la constitución de las capas de la retina. Por lo cual este estudio suministra la base para la interpretación de resultados de la OCT en cuanto a la morfología fisiológica de la fóvea en aves de presa y la condición previa para el registro y calificación de las alteraciones en la fóvea.

Abstract

Image editing and archiving was done using the software program Heidelberg Eye Explorer (HEYEX) version 5.4 (Heidelberg Engineering, Heidelberg, Germany). From the foveae centralis and temporalis, horizontal and vertical cross-sectional images (B-Scans and 3D images) were conducted. This included scans of the deepest spots of the fovea and of the parafoveal eminences at the locations nasal, temporal, superior, and inferior. Additionally, longitudinal- and cross-sections of a retinal comparison area were prepared, coming from the intersection of the eyeball equator (along the blood vessel cascade) and an imaginary extension along the pecten oculi. The measurements in the region of the central fovea, i.e. at the deepest point, consisted of the entire foveal retina thickness, the thickness of the layer complex spanning from the inner nuclear layer to the outer limiting membrane, and the complex from the outer limiting membrane to the outer retinal pigment epithelium resp. to the basal membrane. Furthermore, the depth of the depression was calculated and the retina layering of the central fovea was recorded. In the eminences and in the intermediate region, the retinal total thickness (from the internal limiting membrane to the basal membrane) and the following layers were measured as complexes: the nerve fiber layer together with the ganglion cell layer, the inner plexiform layer and the inner nuclear layer, the outer plexiform layer and the outer nuclear layer, the inner nuclear layer to the outer limiting membrane, and the outer limiting membrane to the outer retinal pigment epithelium resp. to the basal membrane. In addition, nasal and temporal distance, as well as the superior to inferior eminence distance, was measured indicating the width of the Fovea at the retinal surface. Using OCT, we were able to image the fovea in near histologic grade. Similarities and significant morphological differences between species were observable. In all bird species, a somewhat elongated fovea shape was observed. The different fovea types, convexiclivate for the diurnal birds of prey and concaviclivate for the nocturnal types, were clearly distinguishable, as well as differences in size within the diurnal and nocturnal species and between the two groups. In the diurnal types, the fovea centralis was wider and deeper as compared to the fovea temporalis of owls. The fovea temporalis of the diurnal birds of prey was overall less clearly formed and was more similar to the fovea temporalis of the nocturnal types. The layers in the central area of the fovea were reduced up to the inner nuclear layer of the fovea centralis of diurnal birds of prey and the fovea temporalis of owls. In the fovea temporalis of the diurnal types, the reduction was somewhat less pronounced, namely only up to the ganglion cell layer. In all fovea types, the eminences showed the same layers. The total thickness of the retina as well as of the layer complexes varied between the species within the diurnal and nocturnal groups, as well as between diurnal and nocturnal groups. Thus the total retina and the layer complexes up to the outer limiting membrane were thicker in diurnal birds of prey as compared to the nocturnal owls. The complex spanning from the outer limiting membrane to the outer retinal pigment epithelium/basal membrane was, however, thicker in the nocturnal species. In both groups, the inner plexiform layer and the inner nuclear layer were thicker than the outer plexiform layer and outer nuclear layer. However, in the nocturnal species, the outer nuclear and outer plexiform layers were especially pronounced. In the European honey buzzard foveae, the retinal layers could not clearly be distinguished. Using optical coherence tomography, the fovea of birds of prey and owls could successfully be displayed, including the layering of the retina. This study provides an initial basis for the interpretation of OCT results with regards to the physiological morphology of the foveae in birds of prey and basis points for the assessment of changes in the fovea.

Abstract

La tomografía de coherencia óptica (OCT) es un nuevo método de Imagen, el cual ya es entablado exitosamente en la oftalmología humana. La OCT ofrece la oportunidad de mostrar las capas y la morfología de la retina en sección transversal de un modo no invasivo, sin contracción y « in vivo » con una calidad poco inferior a la histología. En la oftalmología aviaria la OCT no se ha usado con frecuencia. La meta de este trabajo a sido calificar la OCT como instrumento diagnostico adecuado para la investigación de la fóvea. El foco principal del trabajo ha sido la posibilidad de representación y presentación de la fóvea fisiológica de las aves de presa y búhos. Se ha examinado la variación morfológica de los dos tipos de fóvea, la F. centralis y F. temporalis en la misma especie y también la variación dentro y entre los grupos aviarios diurnos y nocturnos. Para esto se ha estudiado en un tiempo de 1.5 anos a 56 aves de presa salvajes diurnos y nocturnos de 13 diferentes tipos y 3 especies. Todos los animales pertenecen a la clínica de la universidad Ludwig-Maximilians-Universität en Múnich. En todos los animales que recibieron un examen oftalmológico seguido de un examen de OCT habían indicaciones para tal. Dependiendo del estado físico del paciente y su tolerancia, los exámenes se hicieron despiertos, en sedación con Midazolam (2 mg/kg oronasal) or anestesia de inhalación. Para el examen de OCT se utilizo el modelo Spectralis® HRA+OCT Plus de la Firma Heidelberg Engeneering y para la edición de imágenes y archivamiento el programa software Heidelberg Eye Explorer (HEYEX)en la versión 5.4, Heidelberg Engeneering, Heidelberg, Alemania. De la fovea centralis y temporalis se hicieron cortes de imagen verticales como también horizontales (B-Scans y tomas 3D). Estas contienen scans de la parte mas profunda de la fovea y de la eminencia parafóvea las localizaciones nasal, temporal, superior e inferior. Ademas se hicieron cortes verticales y horizontales de la parte intermedia de la retina cual corta con el ecuador del ojo (en dirección de los vasos sanguíneos) y una línea imaginaria a lo largo del pekten, como uso de comparación. Las medidas implican en la parte central de la fóvea, quiere decir en la parte mas profunda, el grueso retinar completo de la fovea, el grueso del complejo de las capas de la capa granular interior hasta la membrana limite exterior y desde la membrana limite exterior hasta el epitelio pigmentario retinar exterior respectivamente hasta la membrana basal. También se calculo la profundidad del hundimiento y se anotaron las capas de la fóvea central. En la eminencia y en la parte intermedia se midió el grueso retinar completo (desde el limite de membrana interior hasta la membrana basal) y aparte las siguientes capas como complejos: las capa de fibra de nervio óptico y capas de las células ganglionares, la capa plexiforme interior y capa granular exterior, la capa granular interior hasta la membrana limitante exterior y la membrana limitante exterior hasta epitelio pigmentario de la retina exterior respectivamente hasta la membrana basal. Además se midió en las “Eminenzen” la distancia nasal-temporal y superior-inferior, como también lo largo de las fóvea en la superficie de la retina. Atravez de la OCT se pudo representar la fóvea en calidad cercana a la histología. Se pudo mostrar parecidos y también claramente diferencias morfológicas entre las diferentes especies. En todos los tipos de aves se pudo mostrar una forma alargada de la fóvea. Los diferentes tipos de fóvea, convexiclival en las aves de presa diurnas y concaviclival en las especies nocturnas, se pudieron mostrar claramente, como también la diferencia de tamaño en dentro de las especies diurnas y nocturnas y también entre los dos grupos. En las especies diurnas la F. central eran mas anchas y profundas en su dimensión comparadas a la F. temporal de los búhos. La F. temporal de las aves de presa diurnas no estaban claramente desarrolladas y se parecían mas a las F. temporal de las especies nocturnas. La reducción de las capas en la parte central de la fóvea alcanzaba en la F. central en las aves de presa diurnas y en la F. temporal en los búhos hasta la capa granular interna. En la F. temporalis de las especies diurnas solo alcanzaban hasta la capa de las células ganglionares. Las “Eminenzen” mostraban en todos los tipos de fóvea las mismas capas. El grueso de la retina en su totalidad y las capas complejas variaban entre las especies de los grupos diurnos y nocturnos, como también entre diurnos y nocturnos en la misma especie. El grueso total de la retina y de los complejos hasta el limite de la membrana exterior era mas grueso en las aves de presa diurnas que en los búhos nocturnos. Las capas desde el limite de la membrana exterior hasta el epitelio pigmentario exterior / membrana basal de la retina eran por lo contrario mas gruesos en las especies nocturnas. En ambos grupos la capa plexiforme interna y la capa granular interna eran mas gruesas que la capa plexiforme exterior y la capa granular exterior. En los búhos la capa granular y plexiforme exterior estaban claramente mas desarrolladas. En el halcón abejero no se pudo clasificar claramente las capas de la retina como en las otras especies de aves. Por medio de la OCT se pudo mostrar la fóvea de las aves de presa y los búhos incluyendo la constitución de las capas de la retina. Por lo cual este estudio suministra la base para la interpretación de resultados de la OCT en cuanto a la morfología fisiológica de la fóvea en aves de presa y la condición previa para el registro y calificación de las alteraciones en la fóvea.
