Untersuchung des Stellenwertes der Messung spezifischer Antikörper im Liquor cerebrospinalis für die Diagnose der ZNS-Manifestation der Felinen Infektiösen Peritonitis
Investigation of anti-coronavirus-antibodies in the cerebrospinal fluid for the diagnosis of CNS manifestation of feline infectious peritonitis The purpose of the present study was to evaluate the occurence of anti-coronavirus antibodies in the cerebrospinal fluid (CSF) of cats and to assess their diagnostic utility concerning the central nervous system (CNS) manifestation of feline infectious peritonitis (FIP). Anti-coronavirus immunoglobulin G (IgG) was measured in CSF and serum of 67 cats by indirect immunofluorescence assay. Additionally, cell counts, Pandy’s test and total protein were performed in the CSF. Diagnosis of all cats was confirmed by gross and histological examination including the brain. Four different groups were examined: FIP with CNS manifestation (n = 10), FIP without CNS manifestation (n = 13), other CNS disorders than FIP (n = 29) and diseases not associated with FIP and not involving the CNS (n = 15). Anti-coronavirus IgG was detected in the CSF of 18% of all cats (12/67): 6 cats with FIP involving the CNS, 4 cats with FIP not involving the CNS and 2 cats with brain tumours. CSF antibodies were only found in seropositive cats. In addition, only cats with serum titers equal or higher than 1:4096 showed CSF titers. The correlation between serum antibodies and CSF antibodies was statistically highly significant (r=0,652; p=0,000). No statistically significant correlation was found between the erythrocyte count and CSF titers. However, a certain tendency for high CSF titers with increasing numbers of erythrocytes was observed. For all cats of the study (n = 67), testing for anti-coronavirus IgG in the CSF showed a sensitivity of 60% and a specificity of 90%. CSF antibodies had a positive predictive value of 50% for the CNS manifestation of FIP. The negative predictive value was 93%. Calculating only the values of cats with CNS alterations (n = 39), testing for anti-coronavirus IgG in the CSF had a sensitivity of 60% and a specificity of 93%. The positive and negative predictive value was 75% and 87%, respectively. The present study revealed that the determination of anti-coronavirus-titers in the CSF alone does not confirm CNS manifestation of FIP. The statistically highly significant correlation between CSF and serum antibodies indicates a disrupted blood brain barrier or blood contamination. Future studies on CSF anti-coronavirus-antibodies should address the evaluation of the status of the blood brain barrier and the measurement of the intrathecally produced antibody portion., In der vorliegenden Arbeit wurde das Vorkommen von Antikörpern gegen Coronaviren im Liquor cerebrospinalis bei Katzen untersucht und ihre Nützlichkeit zur Diagnosestellung einer ZNS-Manifestation der Krankheit FIP evaluiert. Bei 67 Katzen wurden die Coronavirus-Antikörper-Titer in Serum und Liquor mit indirekter Immunfluoreszenz bestimmt. Zur Liquordiagnostik gehörten außerdem Zellzählung, Pandyreaktion und quantitative Proteinbestimmung. Alle 67 Katzen konnten durch pathologische und histopathologische Untersuchung einschließlich des Gehirns einer der folgenden vier Gruppen zugeordnet werden: FIP mit Manifestation im ZNS (n = 10), FIP ohne Manifestation im ZNS (n = 13), andere Krankheiten des ZNS als FIP (n = 29) und andere Krankheiten als FIP und ohne Krankheit des ZNS (n = 15). Coronavirus-Antikörper konnten bei 18% aller untersuchten Katzen (12 von 67) im Liquor nachgewiesen werden. Neben sechs Katzen mit FIP mit ZNS-Manifestation zeigten auch vier Katzen mit FIP ohne ZNS-Manifestation und zwei Katzen mit Gehirntumoren Antikörper im Liquor. Nur seropositive Katzen hatten Coronavirus-Antikörper im Liquor. Zusätzlich zeigten nur Tiere mit einer Serumtiterhöhe von größer oder gleich 1:4096 Antikörper im Liquor. Serum- und Liquorantikörper waren über eine mittlere, positive Korrelation statistisch hoch signifikant verbunden (r=0,652; p=0,000). Obwohl der Einfluss der Erythrozytenzahl auf die Antikörper im Liquor nicht statistisch signifikant war, traten tendenziell mit steigender Zahl der Erythrozyten auch höhere Titer im Liquor auf. Für alle Katzen der vorliegenden Studie (n = 67) hatte der Nachweis von Coronavirus-Antikörpern im Liquor eine Sensitivität von 60% und eine Spezifität von 90%. Der Nachweis von Liquorantikörpern zeigte einen positiven prädiktiven Wert von 50% für die ZNS-Beteiligung bei FIP. Der negative prädiktive Wert betrug 93%. Bezogen auf die neurologisch erkrankten Katzen der Studie (n = 39) zeigte der Nachweis von Liquorantikörpern eine Sensitivität von 60% und eine Spezifität von 93%. Der positive prädiktive Wert lag bei 75%. Der negative prädiktive Wert betrug 87%. Nach den Ergebnissen der vorliegenden Studie leistet der alleinige Nachweis von Coronavirus-Antikörpern im Liquor keinen Beitrag zur Diagnose einer ZNS-Beteiligung bei FIP. Die statistisch hoch signifikante Korrelation zwischen Liquor- und Serumantikörpern weist auf eine gestörte Blut-Hirn-Schranke oder eine Blutkontamination hin. Zukünftige Untersuchungen zu Coronavirus-Antikörpern im Liquor sollten den Status der Blut-Hirn-Schranke und den Anteil einer intrathekal produzierten Antikörperfraktion evaluieren.
